Names

First Name (or Initial)

• If you selected INDIVIDUAL, enter your FIRST NAME or initial.
• If you selected ENTIRE FAMILY, enter the first name or initial of the head of the household.

Middle Name (or Initial)

• Fill in your MIDDLE NAME or INITIAL.
• If you selected ENTIRE FAMILY, enter the middle name or initials of the head of the household.

Last Name

• Fill in only one LAST NAME.
• If anyone with the same last name is moving to a different address, use a separate form for each person.

Note:  

If you receive mail by more than one name (maiden name, married name, or nickname) please fill out a separate form for each name and check the INDIVIDUAL box.
